Item 12--Reporting cycle

Item 13--Operating injection pressures

  • Report maximum recorded annulus pressure in the “Hydrocarbon or Gas” column for each reporting month.

    • Select code “2” for a well permitted under Statewide Rule 95 or 97 that is undergoing leaching during this reporting cycle.
    • Select code “4” for a well permitted under Statewide Rule 95 or 97 that is operating under temporary alternative monitoring status under section (o) of these rules.
    • Select code “5” for a well permitted under Statewide Rule 95 or 97 that is operating under permanent alternative monitoring status under section (o) of these rules.
